  • This service includes a free phone consultation followed by a visit to your home or to the hospital, where we will undertake an assessment of your health and care needs.

All required paperwork is completed, and the process and the fees and charges are explained. We can also assist with completion of the ‘Request for a Combined Assets and Income Assessment’.

We will advise you as soon as there is an offer of a placement and you will then have the opportunity to view the facility. Age Care Directions facilitate the whole process but it is always your decision whether to accept the offer of a placement.

  • If you are in hospital, ready to be discharged and unable to return home, we are able to update the hospital staff on our progress with finding you a Residential Care placement. This is where our assistance can be very beneficial to you, as often the hospital will only allow you a few days to find a suitable placement.

  • And if you are a couple and both requiring placement in Residential Care, we can assist by obtaining placement for both of you in the same facility.
