Residential Care Placement Service

Age Care Directions provide a full application service for people who are seeking a placement in Low Care (Hostel) or High Care (Nursing Home) accommodation (residential care), either permanent or respite care.

This service is aimed to assist people who are elderly and due to deteriorating health are no longer able to manage within their own home.  We are often approached by family members to assist them with finding their parent(s) residential care.

 


The benefits of using our service include:

  • eliminate stress - we complete the paperwork and follow up phone calls, provide advice and guidance, while finding a placement that meets the needs of the family.
  • simplicity- we are able to simplify the process of applying for residential care by providing guidance and using our many years of experience in aged care.
  • quality care- we recommend facilities where you can be confident you or your family member will receive a high quality of care and be provided with a wide range of activities designed to meet your care needs.
  • opportunity- most high quality Residential Care facilities have long waiting lists, and it can take several months before a vacancy is offered.  Through our application strategy, together with the close liaison we share with residential care providers and the advocacy we provide on your behalf, we have an excellent record of achieving an early placement. This is often within as little as two to three weeks from when we meet with you.
  •  advocacy- we advocate on your behalf, not only in finding a placement for you, but we are also available after the placement to assist with any issues or concerns that may arise, at no extra cost.
  • financial- while not licensed to provide financial advice, we are able to explain the various fees and charges that apply to Residential Care, and we also assist you with completing the various financial documents.  Upon request we do provide you with contact details of financial advisors who have a sound understanding of aged care.
  • other-  we can link you to people in real estate who can assist with sale of your house and disposal of surplus personal assets.   


We offer a free phone consultation and our staff can come and visit you in your home to complete the necessary documentation. We are available to meet outside normal business hours. 

Following our meeting we lodge the applications then provide you with  a list of facilities through which applications have been lodged.  We then update those facilities and advise you of vacancies until a suitable placement is found.

Where we are seeking a placement in a Low Care facility, we recommend facilities that have ‘ageing in place’ or High Care on site, with a view to ensuring there is no need to move again should the person’s health deteriorate.
